- Copy of Birth Certificate (new/first year player with Sartell Youth Hockey. PDF format, NO pics

| 06/01/2013 to 05/31/2015 | 4th & 5th | Squirt/Girls 10U |
| 06/01/2015 to 05/31/2017 | 2nd & 3rd | Mites 8U |
| On or after 06/01/2017 | Pre-K, K, & 1st | Mini Mites 6U |
